If your doctor orders a medication to be given by the nurse, the PARENT/GUARDIAN MUST bring the medication to the Van Wyck Health Office in its original container and it will be kept in the health office until the end of the school year. In JUNE, it is the responsibility of the parent and/or guardian to pick up all medication at the end of the each school year. 

 

If your doctor states that your child can SELF-CARRY and SELF-ADMINISTER their medication, then the student can bring in the medication. The WCSD Medication Form must be used for SELF-CARRY orders. The medication will be checked for correct medication, dosage, and expiration date. It will be labeled with the student name.

 

**Self-carry orders are required for any student, needing any medication, who wants to try out for a SPORT or wants to participate in any after school activity. The Health Office is closed after 2:25 PM daily.
