Please click on link below if your student needs medication to be given during school hours. The top portion should be filled out by a Physician and the consent portion on the bottom needs to be filled out by Parent or Guardian.
An order for medication from the doctors' office can also be faxed to (845) 298-5232.
ALL MEDICATIONS NEED TO BE GIVEN BY PARENT OR GUARDIAN TO THE HEALTH OFFICE. STUDENTS CAN NOT BRING MEDICATION TO SCHOOL!
Medication will not be given in school unless a doctors' order and parental consent form are signed and given to the Health Office. (Doctors' orders and parental consent forms for in-school medication can not be used from previous school years. A new doctors' order and parental/guardian consent form is needed every school year.)
If your student is able to self-carry a medication (ie inhaler) in school, the physician needs to attest on the form or order that the student can self-carry the medication in school and the form or order given/faxed to Health Office.
